Consumerism in Healthcare does not just occur within the walls of the care facility. A large determinant of high HCAHPS scores is directly related to communication of post-care discharge instructions to the patient and family around managing the care plan. Another key factor is then the successful proactive follow-ups to engage with the patient and family post-acute care by a case manager or caregiver. The speaker at this session will discuss the post-acute care patient experience from recent personal experience since HIMSS17. First, he will review with the attendees the results of his work with a nationally recognized tertiary pediatric care hospital. Next, he will review his learnings from collaborating with his sister on the management of their father’s care plan when he was unexpectedly disabled in the last 6 months. In both cases, the quality of care within the facilities were fantastic experiences, but degraded substantially after discharge. In both cases, the family engagement was critical to the success of managing the patient care plan.

Rabin Medical Center

Liver surgery requires understanding of the complex spatial organization of blood vessels, bile ducts and other vulnerable structures inside the liver. Therefore, liver resection is planned using three-dimensional imaging. Typically, surgeons cannot access three-dimensional imaging during surgery, and rely on their memory to navigate through the liver tissue. Microsoft HoloLens is a head-mounted display which projects three-dimensional holographic images on the physical surroundings of the user. We developed a Microsoft HoloLens compatible software that reconstructs liver imaging studies into three-dimensional holographic objects. The software was utilized during liver resection to project a reconstructed holographic image of the patient`s cross-sectional imaging studies. This technology may assist liver surgeons to completely remove liver tumors while avoiding injury to vulnerable structures.

CLEW

CLEW Medical develops a platform for early detection of deteriorations in critical care units. Together with researchers from Mayo Clinic, CLEW used data from 10000 de-identified test patients to Azure cloud to validate and refine a computerized model that could detect Sepsis in a medical ICU. The model utilizes over 600 data points types per patient that are acquired in real time from the EMR.  The model developed by CLEW can create a significant impact on the healthcare industry, predicting life threatening complications in high-acuity patients. Moreover, this model has the potential to be extended to all care settings and be applied to a broad range of complications, with ultimate goal to improving clinical outcomes for large portions of the patient population. The model and system showed good reliability with no down time and ability to process the large amount of data needed for algorithm execution.

Project InnerEye develops machine learning techniques for the automatic delineation of tumors as well as healthy anatomy in 3D radiological images. This enables: 1. extraction of targeted radiomics measurements for quantitative radiology, 2. fast radiotherapy planning, 3. precise surgery planning and navigation. In practice, Project InnerEye turns multi-dimensional radiological images into measuring devices. Project InnerEye builds upon many years of research in computer vision and machine learning. It employs algorithms such as Deep Decision Forests (as used already in Kinect and Hololens) as well as Convolutional Neural Networks (as available in CNTK) for the automatic, voxel-wise analysis of medical images. The technology is designed to be of assistance to expert medical practitioners. The results of our machine learning operations can be readily refined and adjusted by expert clinicians until completely satisfied with the results. The doctors maintain full control of the results at all times. Our algorithms are deployed as intelligent Azure services and they are intended to be consumed by third-party medical software manufacturers. The InnerEye cloud API is designed to be integrated within third-party software, to augment it with Microsoft’s world-leading AI capabilities.
